Diversity Wellness requires everyone who uses our Site to be a registered member. To register for the Site, you will need to provide us with your first and last name, e-mail address(es), a log-in name, a password, ZIP code, sex, and date of birth. You may choose not to provide this information, but you will not be able to access or use our Site if you choose not to provide the information. Individual consumer members who are not sponsored by a corporation will be required to provide credit card information (e.g. a valid credit card number and expiration date, the name on the credit card, and the corresponding billing address) in order to access the Site.
You may choose not to provide this information, but you will not be permitted to access or use those features and tools if you choose not to provide the information. We will use this Personal Information to contact you if necessary or, if you choose to receive e-mail from us or our partners, to send you newsletters, and to inform you about special events that might interest you. Diversity Wellness will not knowingly sell, rent, or trade the personal information we collect. You always have the option to change your mind about receiving e-mail from us. Simply follow the unsubscribe directions in the footer of our newsletters and promotional e-mails.
You may also change or delete your personal information at any time by going to the Profile or My Account section of the Site and submitting the appropriate changes. If you provided personal information within a particular application, such as the Personal Profile or Walking Program, you can change or delete that information by visiting those applications and submitting the appropriate changes.
If you choose to cancel your membership, your personal information will become "inactive" in our database. If at any time, you wish to permanently remove any personal information from our system, please contact us directly at the e-mail address provided at the end of this policy.
Diversity Wellness may also collect demographic information from users, such as age, gender, and areas of interest, as well as information regarding the "IP address" that is used to identify your computer on the Internet and information obtained by tracking the "clickstreams" from your usage of our Site. We use this information to improve our Site, by measuring interest, traffic, and usage patterns. We may share this information with third parties, in a non-personal, aggregate form only. If your membership is sponsored by a corporation or other third party, we may share information with that party in aggregate form only. For example, we may report to a corporation the number of its employees using our service, or the frequency of their collective use. This information will not identify you personally, and we will not provide any Personal Information to any third party. We do not share personally identifiable information with your employer.
Online Shopping
If you make a purchase on the Site, or register as a consumer user, you will need to provide us with more detailed Personal Information so that we can complete your transaction. This includes a valid credit card number and expiration date, the name on the credit card, the corresponding billing address and the shipping address, if different (as well as other shipping addresses if you are sending any items as gifts), and your e-mail address. We may provide the shipping information to one or more of our agents (for example, a fulfillment service). These agents have committed to abide by our Privacy Policy and will not share or release your Personal Information for any reason whatsoever.
When registered Diversity Wellness members make a purchase on the Site, we will store some information, such as credit card numbers and billing and shipping addresses, to speed up subsequent transactions. Thereafter, Diversity Wellness will never ask for your account information and password by e-mail.
